Coronavirus information update 25/03/2020

Due to the changes made to the NHS advice regarding COVID-19 on Monday 23rd March 2020, Doctors of the World have updated the COVID-19 translated guidance at: https://www.doctorsoftheworld.org.uk/coronavirus-information/

It is now available in English, Albanian, Arabic, French, Portuguese, Spanish, Turkish, Polish, Romanian, Russian. The following languages will also be available soon: Bengali, Dari, Farsi, Hindi, Kurdish Sorani, Mandarin, Pashto, Urdu, Vietnamese, Tigrinya, Amharic, Malayalam, Somali, Bulgarian, Latvian, and Hungarian. 

The guidance is based on NHS and the UK Government’s updated advice and health information, and we have produced translated versions thanks to the support and assistance of the British Red Cross, Migrant Help, Clear Voice, and the Eastern European Resource Centre. We really hope it will help to ensure this important guidance reaches migrant and asylum-seeking communities in the UK.

COVID 19 update - Appointments at the surgery from 11/3/2020 until further notice:

From 11/3/2020 we will be providing telephone triage appointments with our doctors until further notice. This is to minimise the risk to patients and staff to the coronavirus and avoid any unnecessary contact. Patients may be contacted by our reception team to change face to face appointments to telephone consultation with doctors or nurses where a face to face appointment is not needed.

We provide flu vaccinations to eligible patients during the flu season from September to March. Please contact us for more information or to book an appointment.

PLEASE NOTE: UNFORTUNATELY THE SURGERY NO LONGER PROVIDES EXTENDED HOUR CLINICS. WE WILL HOWEVER CONTINUE TO MAKE EVENING APPOINTMENTS AVAILABLE UNTIL 6.30PM ON MONDAYS AND WEDNESDAYS FOR COMMUTERS.

PLEASE NOTE: The GP surgery is based in Church Elm Lane Health Centre which also houses other community services including Phlebotomy, Health Visitors and Talking Therapies which are NOT part of the GP. Please contact 0300 300 1790 to access these services or ask about any further enquiries.

With patients' needs at the heart of everything we do, our website has been designed to make it easy for you to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register. You will find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.

New Patient Registration

If you live within our practice area (see above) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID such as a passport. Families with children also need to provide a record of immunisation for each child.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

PreRegistration

When you register you will also be asked to fill out a health questionnaire. This is because it can take a considerable time for us to receive your medical records.

Please note that you will not be registered at the surgery until you have returned all the relevant I.D that is requested with the relevant paperwork that needs to be completed for the surgery records. You will then be seen by our Health Care Assistant for your New patient Medical check.

PLEASE NOTE:  DURING THE CORONAVIRUS PANDEMIC YOU CAN STILL REGISTER WITH THIS SURGERY. PLEASE DOWNLOAD ALL APPLICATION FORMS AND EMAIL/POST THEM TO US WITH PROOF OF YOUR ADDRESS AND ID. We will then contact you regarding the completion of your registration. Please make sure you give us current contact details so we can contact you.
